There are a number of tests you are required to undertake prior to your appointment with your Healthcare Practitioner.  Please add your data/results to the My Results form, which will be sent to you upon booking.

Why participate in your Tier 2 Virtual Health Assessment?

It is important to keep track of your health and wellbeing. Tier 1 health assessment identified you having a BMI greater or equal to 30 and / or a waist circumference greater than 88cm (female) / greater than 102cm (male). It is important to address this because your risk of some health problems increases according to where you store your body fat, as well as by your weight.

With a higher BMI and waist circumference, this indicates that you are carrying too much fat around your middle (waist) and this can increase your risk for conditions such as:

  • Heart disease
  • Type 2 diabetes
  • Cancer
  • Stroke

There are many checks you can do from the comfort of your own home such as those you carried out in Tier 1. By undertaking the Tier 2 tests (listed below) and following up your results with your Healthcare Practitioner, we will be able to provide you with practical solutions and advice to assist further in your health and wellbeing, and guide you if you need to seek medical advice, for which you will have some useful information.

What checks will be carried out in Tier 2?

You will need to conduct these tests prior to your virtual Tier 2 appointment with your Healthcare Practitioner.

  • Blood glucose testing (can indicate diabetes)
  • Cholesterol testing (high cholesterol is a risk factor for heart disease)
  • Thyroid testing – females only (underactive thyroid may be linked with a higher BMI and waist circumference).
  • Prostate testing – males only (PSA test can indicate prostate cancer)

This page will take you through all the tests you need to undertake.

What to expect on the day of your appointment

30 minute virtual health assessment

You will have a 30 minute virtual health assessment appointment with your Healthcare Practitioner. Please be prompt for your appointment as we are unable to run over time, there will be three attempts to contact you.
Your Healthcare Practitioner will introduce themselves to you, review and explain your results and discuss some personalised advice and solutions.
At the end of your virtual health assessment appointment, your Healthcare Practitioner may refer you to your General Medical Practitioner or Healthcare Provider.
Your health assessment report will be updated and sent to you on the email address you have provided on the same day as your appointment.

What Pre-assessment checks do I need to do before my virtual appointment?

Blood Sugar (diabetes) and Cholesterol Testing

These tests will be in a pack together and there will be 2 tests each.

Cholesterol – This test requires some blood (2 drops) through a ‘skin prick test’. All the instructions will be in the test kit including how to interpret the results. It is important to read the instructions before you carry out the test.

Diabetes – This requires a small amount of urine to complete the test – this tests both for blood sugar and ketones in your urine. All the instructions will be in the test kit including how to interpret the results. It is important to read the instructions before you carry out the test.

When you have undertaken these tests, note down the results immediately on the My Results form.

Self checks – Females only

Thyroid Testing

The major hormones the thyroid gland makes are called T4 or thyroxine and T3 or triiodothyronine. The production of these hormones are stimulated by another hormone called thyroid stimulating hormone (TSH). This test detects thyroid stimulating hormone (TSH).
The thyroid hormones are very important as they control the rate at which the body uses and stores energy from the food we eat (the metabolic rate).

This test requires some blood (2 drops) by undertaking a ‘skin prick test’. All the instructions will be in the test kit including how to interpret the results. It is important to read the instructions before you carry out the test.

When you have undertaken this test, note down the results immediately on the My Results form, attached to the email.

Self checks – Males only

Prostate (PSA) Testing

This test has been designed so that you can identify the levels of P.S.A. (Prostate Specific Antigen) present in your blood. P.S.A. is a chemical that appears in your bloodstream in higher concentrations when there may be a challenge with the prostate gland. This test requires some blood (2 drops) by doing a ‘skin prick test’. All the instructions are within the test kit including how to interpret the results. It is important to read the instructions before you carry out the test.

When you have undertaken these tests, note down the results immediately on the My Results form attached to the email.

You have now completed all the data/tests required for your virtual health assessment, Tier 2

Please ensure you have your data/results available for your Healthcare Practitioner on the day of your appointment.

Although self-tests are a useful tool, they are based on a one off result and are not an alternative for NHS screening programmes. A number of factors may provide a positive or negative result and your virtual health assessment consultation is based on the results provided by you. Always consult a medical practitioner if you have any health concerns. A GP will conduct an appropriate assessment and advise further investigations or examinations where necessary.
